When can I expect to receive my reports?
Approximately 6 weeks after the end of each quarter (mid-May, mid-August, mid-November, mid-February).
Who receives reports at my centre?
The Chairperson and Coordinator as designated by the centre.

How do I get my MacsFlirt?
Phone, fax or email the project office. The program is offered to every centre at the time of enrollment.
I am having trouble loading MacsFlirt.
We can try to help you via a phone call from the project office. We do know certain “tricks” for different operating systems that we can give you. If you are having a lot of trouble, contact your IT department and we can work with them.
I have just installed my latest data CD. Why have my data values changed?
Previous reports are updated to ensure that you have the most complete data possible. The change in values indicates that some of your forms were received after the final scan day.

Forms
Can I submit photocopies?
No. Photocopies cannot be read by the scanner.
When can I order new forms?
Anytime. If you find you are running low, contact the project office. Regular mail outs are done every quarter.
Can I use pencil?
Yes, but please press hard and use white-out to erase any mistakes.
Is there any pen colour that I cannot use?
Red and green don’t always show up clearly on the scanner.
Do I have to send my forms every week?
No. If your centre submits less than 25 forms per week, you can send your forms every two weeks. Please make sure that they are at the project office prior to the final scan date.
Do I need to keep a photocopy of my forms?
No.
Does the project office keep a copy of the forms?
Paper copies are kept for one quarter. Electronic versions are kept on CD-ROM.
What does the date on the bottom of the form mean?
That is the revision date. The most recent versions of the form were created in 2005.
Why do you “revise” the forms?
Forms are revised to ensure that we are capturing the latest best practice recommendations.
What is the “final scan day”?
The last day that we submit forms for a quarter. It is usually the first Friday of the full week after the end of a quarter.
Why do I receive Data Clarification Forms (DCFs)?
DCFs complete the missing and inaccurate information from your forms. Most of the information can be found in your logbook.

Can we change our coordinator or chairperson?
Yes, please alert the project office in writing.
Is there any cost to participate?
No. All supplies and data are provided at no cost.

Will the project office share my data with a third party?
No.
My hospital is “linked” with another hospital. Can we get combined reports?
Yes, with written agreement from both centres, we can create combined reports.
