Case Studies
Below are a series of case studies for each phase. The phase is listed first followed by a brief description of the type of patient case reviewed. Please check back soon as more case studies are being added. If you have any interesting cases that you would like to share with others, please contact the Macstrak Project Office and someday your case may be "up in lights"!
Medication List
Below is the most recent PDF version of a medication list in alphabetical order by drug type, including both generic and brand names. This list can also be found in the procedure manuals. Please feel free to post it in your unit as a quick reference. It will be updated as new medications are brought to market. Check back often!
In "Other" Words
When filling out an admitting or discharge diagnosis on the form, the box marked “Other” is frequently used. We are finding that many of the diagnoses given actually belong to one of the diagnostic groups already listed on the form. To help maintain your data’s integrity, we have taken the most common diagnoses and shown how they should be recorded on the form. Feel free to print this sheet and post it in your unit. Should you have any questions about diagnoses not mentioned, please contact the Macstrak Project Office.
Newsletters
Below are a list of all the newsletters. Click on the title to see the newsletter or click on the corresponding graphs link to download the graph group for the newsletter. The graph group will then need to be loaded into your MacsFlirt program (using Data Tools -> Import) to be viewed. You may receive a message stating: "Import of reports completed successfully but some graphs not imported because relevant reports not available," unless your hospital particiaptes in all three phases (ER, CCU and Ward) of the project. Please note that Graph Groups began in the fall of 2006 so there are no graph groups prior to Newsletter V13I3.
Presentations
Below are PowerPoint slide shows that have been used by the Project Office as teaching tools at various meetings. Please feel free to use them at your hospital as review slides. Some can be used in a group setting but others, like the ECG Interpretation Presentation, could be used by an individual as well. We will be updating them often so check back frequently!

Customizing Your Macstrak Form - Optional Groups and Fields
Want to take a closer look at your diabetic population? Need to know how long patients are waiting to be transferred out of your unit? By using Optional Groups and Field sections on the Macstrak form your unit can address these, and other, centre-specific data needs. The Macstrak Project Office has developed a manual for each phase, describing the function of both optional groups and fields describing how the track various types of data and as well as providing examples of the reports your centre will receive. Below is a link to a PDF version of a manual that will assist you in using your Optional Fields and Groups to your best advantage. Please feel free to make a copy and start tracking an issue specific to your unit!
Privacy Statement
Standards for ethics review, and legislation governing health care information continue to evolve. In 2004, the Ontario government passed the Personal Health Information Act, dealing with the collection, use and sharing of personal health information. Other provinces are also providing legislation. Both the lay press and medical journals are discussing the need for ethics review of data collected for quality assurance. We continuously review our practice with health information and take every reasonable precaution to safeguard all information. Please see About Us - Ethics Review and Privacy Statement.
We also believe that a part of best practice in the management of health information is communication with patients. The Macstrak Project Office is offering centers a one-page information sheet addressed to patients advising them of the purpose of the project and their rights. We would like to encourage hospitals to include a document addressing this issue in the materials offered patients at the time of admission to your unit, or posted in your CCU, ER or Ward service. Patients, who choose to opt out of the project, can contact their bedside nurse who in turn will inform the Macstrak coordinator at your centre.
